To simplify your first visit, please fill out our New Patient Form* online prior to your appointment or arrive 10 minutes early to fill it out in office.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
As a courtesy to all our patients, we will submit claim forms and estimates directly to your insurance company on your behalf. We will accept payment from your insurer provided they allow you to assign payment directly to the dental office.
In order to do this, we require complete information at the time of the appointment which includes: insurer's name and mailing address, group/policy/contract number, and ID/certificate number.
Rarely is there full or 100% coverage and there will be a co-payment that patients would be responsible for.
Payment is required at each appointment. We accept cash, debit, Visa, Mastercard, and American Express.
Putting your health “on hold” for financial reasons is not something that is in your best interest. Unfortunately, neglecting or ignoring needed dental care can actually be more expensive in the long run.
If your oral health situation is allowed to continue to deteriorate your problems usually become more complex and more expensive the longer you wait.
